Order a membership card

Replacement cards

You can order a replacement card via:

Online Member Services

  • Log in
  • Go to My membership
  • Select Order membership card
  • Select Name (who the card is for)
  • Click Request card

Nurses & Midwives Health app

  • Log in
  • Tap Membership
  • Select Order membership card(s)
  • Select member’s name
  • Check mailing address is correct
  • Click Order card(s)

New cards

To order a card for a member who hasn’t had one before, please contact us.

Receiving your card

Please allow up to 10 working days for new cards to arrive at the postal address we have on file.

In the meantime, you can submit Extras claims via the Nurses & Midwives Health app or Online Member Services.
